To ensure that everything runs smoothly on the day of your wedding, we require all our clients to fill out the following form. There will be a total of 3 PARTS which will require information about your wedding day, shot requests and selected vendors. Please double check to make sure everything is correct and all required fields are filled out before submitting the form at the end of Part III. After submission, a copy of your form will be automatically CC’ed to your email for your records. Should there be any changes after your submission, please inform us via email ASAP. Thank you!

This is our TYPICAL SHOT LIST for a wedding. If there are any shots that are NOT on this list that you'd like us to capture, please list them in the appropriate box below. This will help us prepare and keep an eye out for person(s)/things that are important to you. We cannot guarantee that we will get everything, but we'll do our best!

PART III : VENDORS & ATTIRE

As photographers, we try our best to capture all the little details of your wedding day. Whenever possible, we would like to give your vendors due credit for their work, and possibly provide them with photos for their portfolio. We would appreciate it if you could fill out as much information as possible about your selected vendors, so we can credit and refer them wherever necessary.
 
If your vendor is a family member or friend who would like to be contacted for future opportunities, please include a phone number, email or instagram handle so we can refer them accordingly.

  • A list of rules/guidelines for the Ceremony/Reception Venue (if applicable)
